• Your child must meet the North Carolina Immunization Guidleines
• All students enrolling in a North Carolina public school for the first time must have a physical assessment for enrollment unless a valid exeption applies.

Ask your doctor for a Certificate of Immunization (vaccine record)

Students who will enter the 7th grade this year must receive one booster dose of Tdap vaccine and one dose of Meningoccal (MCV) vaccine for individuals who have not already received those vaccines.

Students who will enter the 12th grade must receive a second dose of Meningococcal (MCV) vaccine if they have not already received this dose.  

You must provide the school with your child's up-to-date immunization record and health assessment within 30 calendar days from the first day of attendance. If this is not received by the 30^th day of school, your child will not be allowed to attend school until this information is received.
